What you will need…

To make sure the notarization process goes smoothly, please be sure you have these items before I arrive to serve you:

• The document to be notarized. I am able to provide the additional service of printing the document(s) for an additional charge. The document(s) must be emailed to me at least 4 hours before the appointment.
  • $2 printing fee, plus .50 per page (up to 10 pages then negotiable)
  • I can also provide other administrative services for additional fees. (scanning documents, sending copies via email, FedEx etc.) Fees negotiable.
• Usually, the document will have a pre-printed notarial certificate at the bottom that I will fill in. If your document does not already include this certificate, you may wish to ask the organization or agency to which you’re submitting the document exactly what type of notarization is needed. The two most common types of notarization are an oath and an acknowledgment. Per state law, I cannot advise which type of notarization may be needed. If the document does not have the proper notarial wording/certificate, I can provide the proper wording certificate for either type as a separate stamp to complete the notarization.
• I will need satisfactory evidence of the identity of an individual appearing before me. Per state law, I need to identify the individual by means of:
  • (I) A passport, driver’s license, or government-issued nondriver identification card that is current or expired not more than one year before performance of the notarial act; or
  • (II) Another form of government identification issued to the individual that is current or expired not more than one year before performance of the notarial act, contains the signature or a photograph of the individual, and is satisfactory to the officer (notary).
• Witnesses (if needed). Examine the document to determine if witnesses are needed. If your document requires witnesses, you will need to have witnesses available at the time of signing. Hospital and nursing home staff and not able to perform this task. On occasion, I am able to provide witnesses at a cost of $30 per witness, however this is not guaranteed.
• The person signing must be of sound mind and I must be able to confirm that they know what they are signing and are signing the document of their own free will. I may refuse to perform the notarial act per C.R.S. 24-21-508.
